Artificial Sweeteners and Cognitive Decline: What a New Study Found
For decades, artificial sweeteners were sold as the guilt free swap: all the sweetness, none of the calories, none of the blood sugar spike. That reputation has taken a hit over the past year. A large, long running study published in Neurology, the journal of the American Academy of Neurology, found a link between high intake of several common sweeteners and a faster rate of cognitive decline. It is worth walking through what the study actually found, what it does not prove, and what a reasonable person should do with that information.
The Study, in Plain Terms
Researchers at the University of Sao Paulo followed nearly 13,000 middle aged adults, average age 52, for about eight years. At the start of the study, participants filled out detailed food questionnaires that let researchers estimate how much of seven different low and no calorie sweeteners they consumed: aspartame, saccharin, acesulfame K, erythritol, xylitol, sorbitol, and tagatose. Participants also took cognitive tests three times over the study period, covering memory, verbal fluency, working memory, and processing speed.
The researchers split people into three groups based on total sweetener intake. The lowest group averaged about 20 milligrams a day. The highest group averaged 191 milligrams a day, roughly the amount in a single can of diet soda. After adjusting for age, sex, blood pressure, and cardiovascular disease, the people in the highest intake group showed a 62 percent faster decline in overall cognitive performance than the lowest group, an effect the researchers described as equivalent to about 1.6 years of extra brain aging. The middle intake group also declined faster than the lowest group, just less dramatically.
Six of the seven sweeteners studied, aspartame, saccharin, acesulfame K, erythritol, sorbitol, and xylitol, were associated with faster decline. Tagatose was the exception; it showed no link to cognitive decline in this data.
Who Seemed Most Affected
Two patterns stood out. First, the association was present in adults under 60 but not in people older than 60, which is a bit counterintuitive if you assume cognitive risk only compounds with age. Second, the link was noticeably stronger in people with diabetes than in people without it, which researchers flagged as worth watching given how often sweeteners are recommended specifically to people managing blood sugar.
What This Study Does Not Show
This is the part that gets lost in headlines, so it is worth stating clearly. This was an observational study, not a controlled trial. It can show that sweetener intake and cognitive decline moved together in this population; it cannot show that one caused the other. The study's own lead author, Dr. Claudia Kimie Suemoto, was explicit about this limitation when discussing the results publicly.
There are a few reasons the connection could be indirect rather than causal. People who consume large amounts of artificially sweetened products may differ from low consumers in ways the study could not fully account for: overall diet quality, existing metabolic conditions that led them to choose sweeteners in the first place, exercise habits, or other lifestyle factors. It is also possible that people already on a path toward faster cognitive decline gravitate toward sweetened diet products for reasons connected to that same underlying health picture, rather than the sweeteners driving the decline directly. Industry groups, including the International Sweeteners Association, pointed to exactly this kind of confounding when the study was published, and the researchers themselves acknowledged it as a limitation rather than disputing it.
It is also a single study population: middle aged and older Brazilian adults, with intake estimated through self reported questionnaires rather than direct measurement. Self reported diet data carries real error, and results from one population do not automatically generalize to every diet and demographic.
Why It Is Still Worth Taking Seriously
None of those caveats make the finding meaningless. This is one of the largest and longest studies to look specifically at sweeteners and cognition, and it adds to a growing body of research raising questions about the long term safety profile of these additives, separate from their well established benefits around calorie reduction and blood sugar management. The size of the effect, and the fact that it tracked with a plausible dose response pattern (more sweetener, more decline), makes it harder to wave away as noise, even without a clear causal mechanism nailed down yet.
Researchers have proposed a few biological pathways that could explain a real effect if one exists, including changes to the gut microbiome that sweeteners are known to cause, and downstream effects on metabolic and vascular health that could plausibly affect the brain over time. None of this is settled science. It is an active area of research, not a closed case.
What a Reasonable Response Looks Like
You do not need to panic about diet soda, and you do not need to treat this as proof that sweeteners are quietly causing dementia. What is reasonable is treating artificial sweeteners the way you would treat any other food additive with a mixed and evolving research record: worth moderating rather than either avoiding entirely or consuming without a second thought.
A few practical takeaways:
Pay attention to total daily intake, not just one product. The highest risk group in the study was consuming sweeteners across multiple sources throughout the day, not just an occasional diet soda. If your sweetener intake comes from soda, coffee creamer, protein bars, flavored yogurt, and sugar free gum all in the same day, the total adds up faster than it feels like it should.
If you have diabetes, this is worth a conversation with your doctor rather than a DIY decision. The stronger association in people with diabetes does not mean sweeteners are unsafe for blood sugar management; it means the tradeoff deserves a more informed look, since sweeteners are often specifically recommended in that population.
Consider variety over reliance on one sweetener. Given that six of seven sweeteners tested showed a similar pattern while tagatose did not, there is no single obviously safe substitute to switch to. Rotating sources, or simply reducing overall reliance on any sweetened product, artificial or otherwise, is a more defensible strategy than swapping one sweetener for another based on this study alone.
Watch for follow up research rather than treating this as final. Studies like this typically prompt controlled trials and mechanistic research that either strengthen or weaken the association over the following few years. This is a good moment to note the finding and stay moderately cautious, not to overhaul your diet on the strength of one study.
